Whenever I introduce a new patient in my clinic to our high-powered Photobiomodulation panel, the most common immediate question is: "Do I have to take my shirt off, or can the light just go through my clothes?"
TL;DR: Bare Skin is Mandatory
- No, it does not work through clothes. Light either reflects off light-colored fabrics or is absorbed as heat by dark fabrics. It will not reach your body.
- Transmission is Key: For therapeutic photons to reach your mitochondria, there must be absolutely nothing standing in their way.
- Micro-Barriers Matter: Sunscreen, thick lotions, and makeup containing zinc or titanium will also block the light from penetrating your skin.
It is a totally fair question. We are conditioned to think of light like x-rays or microwaves, which effortlessly pass through solid objects.
The definitive clinical answer is: No, red light therapy does not work through clothes.
As a Certified Athletic Therapist (CAT(C)) who relies on precise dosing for tissue repair, I cannot emphasize this enough. If you are wearing clothing over the target area, you are completely wasting your time and your money. Here is the exact physics behind why.
The Physics: Reflection, Absorption, and Transmission
When photons of light leave your panel and travel toward your body, they interact with matter in three specific ways: Reflection, Absorption, or Transmission.
1. Reflection (The Bounce)
If you wear a light-colored or white t-shirt, the fabric acts like a mirror. Instead of entering the fabric, the vast majority of the 660nm red light and 850nm near-infrared light hits the shirt and bounces right back off into the room. Your body absorbs exactly zero medical benefit.
2. Absorption (The Trap)
If you wear a dark or black t-shirt, the physics change. Dark colors absorb light energy. The photons leave the panel and crash into the black fabric, where they are trapped and converted into heat. Your shirt will get very warm, but again, the light never reaches your skin.
3. Transmission (The Goal)
The only way red light therapy works is if the photons transmit directly into the target tissue (your skin, muscles, and joints). For the photons to penetrate the dermis and reach the mitochondria, there must be absolutely nothing standing in their way.
What About Makeup, Sunscreen, or Lotions?
Clothing is obvious, but microscopic barriers also ruin treatments.
If you are using a premium device like a Rouge or Kala panel specifically for anti-aging, wrinkle reduction, or acne on your face, you must treat a completely clean face.
- Sunscreen: Sunscreen is literally designed to block and reflect light (UV). It will drastically impede the penetration of therapeutic 660nm and 850nm wavelengths.
- Foundation/Makeup: Thick foundations contain minerals (like zinc oxide or titanium dioxide) that physically block light from entering the pores.
- Thick Lotions: Heavy oils and creams create a refractive barrier on the skin's surface.
Clinical Rule: Wash your face, dry it completely, do your 10-15 minute session, and then apply your skincare serums immediately afterward, while the circulation is peaked.
Bare Skin is Mandatory
The simple truth is that Photobiomodulation requires direct skin contact with the light. If you are treating a sore lower back, your shirt must be pulled up. If you are treating a knee, you must wear shorts. If you are doing full-body systemic treatments for inflammation or testosterone production, you need to be completely naked.
Maximize Your Investment
Don't buy an expensive clinical panel and then block all the medical benefits with a cotton t-shirt. Strip down, let the light hit your cells directly, and reap the massive rewards of cellular energy production.
